What kinds of Leave are available and how do I know if I am eligible?
Board-approved leaves without pay are available in the following categories: Parental, Personal, Professional, Illness of Self or Relative and Military.  Eligibility to take a leave is determined by the provisions of the applicable Board rules and/or collective bargaining agreement.   Detailed information regarding leaves of absence and eligibility can be found in the Request for Leave of Absence Without Pay application (FM-2763). 
When should I submit my leave application?
Except in the case of an unforeseeable emergency, the employee should apply 30 days prior to the day in which the leave is to begin.  Requests for Personal Leave from employees in the UTD bargaining unit must be submitted 30 working days prior to the commencement of the subsequent school year.
How do I apply for a Leave under the Family Medical Leave Act (FMLA)?
SSchool Board policies and provisions of the collective bargaining agreements are much more generous in terms of extended leaves than the regulations of the FMLA; therefore, FMLA is subsumed in the applicable types of leaves.  However, if you need an intermittent leave of absence under the FMLA you must submit the Intermittent Leave Request Medical Certification Under the FMLA form (FM-7380) to your supervisor.
I am pregnant and I would like to continue working as late in my pregnancy as possible, when should I start my leave?
Once your health care provider has certified, on the Medical Documentation form (FM-6030), your estimated date of confinement you may begin your leave whenever you wish.
My worksite administrator has signed my leave application. Does this mean that my leave has been approved?
No.  The Leave office is the adjudicator of the leave request.  You are not considered approved until you have received a leave approval notification via email at your Dadeschools account.
Do I need to seek approval from the Leave Office if my leave is less than 30 working days?
The Leave office adjudicates leaves that are 31 working days or more.  Leaves less than 30 working days are handled by the work site.  However, there may be  some F&MLA issue with leaves less than 30 days and the Leave office can provide guidance in such instances.
Can I work if I am on a Board approved leave?
The pursuit of other employment while on leave is considered a violation of the conditions of leave unless specifically requested and approved. Any request for permission to work while on leave must be sent in writing to the Leave Office along with supporting documentation for review.
Will I receive retirement credit for the period I am out on leave without pay?
No, unless you are returning from a military leave of absence.  However, under the Florida Retirement System (FRS) Pension Plan, up to 24 months (2 years) of retirement credit may be purchased for authorized leaves of absence.  A member must return to the employment of an FRS employer, in a covered position, immediately upon termination of the leave. Will I be covered with insurance benefits while on leave?
Insurance coverage is provided according to bargaining unit and type of leave.  For more information, please refer to the Leave Information sheet attached to the Leave of Absence application or contact the Employee Benefits office at 305-995-7163.
I am on a leave. What do I do if I wish to extend my leave of absence?
Approximately sixty days prior to the expiration of your leave, you will be mailed a Letter of Intent to complete and return. You may also send in a written request to extend to the Leave Office with the appropriate supporting documentation at least 30 days prior to the expiration of your leave.
Can I return early from my leave of absence?
An employee can only return early during the first twelve weeks of an illness of self, illness of family or Parental leave. UTD bargaining unit employees can return early from an illness of self leave, after giving reasonable notice and providing an unconditional release.
